Study Protocol: Prospective collection of samples to enable the development of Natera screening assay for Early cancer detection PROCEED - Colorectal Cancer Screening (“Study”)

Thank you for your interest in the above referenced clinical research study. Walgreens Health (“Walgreens”) and its service providers, including Curebase, Inc. (“Curebase”) (collectively referred to herein as “we” or“ our”) are performing research evaluation services for the Study, which may include reviewing screening criteria to determine eligibility for participation in the Study, conducting clinical research pre-qualification activities, and contacting you about the Study (the, “Services”).
These Study Privacy Terms (the, “Terms”) let you know how we collect, use, share, and protect information about you for the Services. By interacting with the Services, you consent to the practices described in these Terms. We will notify you of material changes to these Terms by posting on the Curebase platform that the Terms have been revised and by updating the effective date of the Terms.
These Terms apply specifically to your use of the Services and the collection of your personally identifiable and health information necessary to carry out the Services.
In addition, the Curebase platform used for the Services has Walgreens’s and Curebase’s general Privacy Policy available here and Terms of Use available here.
When Walgreens is not providing the Services, its information collection and use practices are described in its Online Privacy and Security Policy and HIPAA Notice of Privacy Practices.
When Curebase is not providing the Services, its information collection and use practices are described in its Privacy Policy and Terms and Conditions.
When you interact with Natera outside of the Study and these Services, its information collection and use practices are described in its Privacy Policy and Notice of Privacy Practices.
Information We Collect for the Services
The information about you we collect for the Services, including when you complete screening questionnaires, consent forms, and surveys related to the Study, includes:
● Personal identifiers, such as your name and date of birth;
● Contact information, such as your address, telephone number, and email address;
● Demographic information, such as your gender, income and age;
● Health information, such as information you share related to a health condition.
We also collect information using “cookies” and similar technologies to support the function of the Curebase platform. “Cookies” are small data files that are sent from a website’s server and stored on your device. The information we may collect using cookies and similar technologies includes:
● Device and connection information, such as Internet Protocol (IP) address, device identifiers and mobile network information;
● Curebase platform account information, such as username and password;
● Statistics on your interaction with the Curebase platform, such as page views and clicks;
● Information about the technical operation and performance of the Curebase platform, such as web traffic information and technical performance issues.
You can determine how to manage cookies and similar technologies that may be transferred to your device, including disabling the technologies, by visiting the Options/Settings section of most internet browsers. You can disable cookies through your browser setting, but please note that disabling cookies may impact some of the platform’s features and prevent the platform from operating properly.
How We Use Your Information for the Services
We may use your information for the following purposes:
● To provide the Services, including to facilitate your activities on the Curebase platform or for the Study consistent with your consent;
● For internal operations, including to improve the Services, analyze statistics and metrics regarding use of the Curebase platform, and perform our business activities related to the Services;
● To prevent fraud and other harm, including to detect, prevent or investigate potential security breaches;
● To comply with law or if necessary and appropriate under applicable law, including to respond to requests from government authorities, comply with legal process, and enforce our terms and conditions.
How We Share Your Information for the Services:
We may share your information as follows if permitted by applicable law:
● Within Walgreens and Curebase, such as subsidiaries and affiliates involved in providing the Services;
● When we work with third party service providers that perform tasks on our behalf to provide the Services;
● When required by law or to protect our legal interests, such as to respond to requests from government authorities and enforce our terms and conditions;
● When we work on business transactions, such as providing transition of operations to another entity; and
● When you provide consent.
We do not sell, rent, or loan your personally identifiable information.
How We Protect Your Information for the Services:
We recognize the importance of maintaining the security of your information. We use reasonable security measures, including administrative, technical, and physical safeguards to protect your information.
Withdraw from Enrollment Consideration
If you change your mind and no longer wish to be considered for enrollment in the Study, you can contact us by telephone toll free 1-800-371-5450 Monday – Friday between 8am – 8pm Eastern Time or email the study team at clinical-trials@walgreens.com.
